This research aims to analyze the implementation of digital transformation in public services at Kabila Community Health Center and identify supporting and inhibiting factors affecting the effectiveness of digital-based healthcare services. This study employed a qualitative descriptive approach through observation, in-depth interviews, and documentation. The findings reveal that digital transformation improved administrative efficiency, accelerated patient registration, strengthened healthcare data management, and expanded public access to healthcare information. Nevertheless, several obstacles remain, including limited technological infrastructure, unstable internet connectivity, and unequal digital literacy among employees and communities. The study further indicates that organizational readiness and human resource adaptability significantly influence the sustainability of digital public services. Therefore, strengthening digital infrastructure and improving digital competencies are essential to optimize sustainable digital healthcare services
