Abstract

This study aims to integrate HIRARC (Hazard Identification, Risk Assessment and Risk Control) and JHA (Job Hazard Analysis) to identify, assess, and control occupational safety risks in pipeline flushing activities in the upstream oil and gas sector. This research employed a descriptive quantitative approach using a case study at PT Alfaja Teknika Mandiri in October 2025. Data were collected through structured observation, semi-structured interviews, document review (SOPs, permits to work, risk registers), and literature study. The research instruments included HIRARC forms, JHA worksheets, and observation checklists. The results indicate that 12 risk groups obtained average likelihood, severity, and frequency scores ranging from 3.3 to 3.9 (scale 1–5), categorized as moderate to high risk. Priority risks include loose hose/pipe connections, over-pressure during flushing, slip/trip hazards, chemical exposure, electrical risks, oxygen deficiency, and human error. The integration of HIRARC and JHA provides structured and practical operational controls aligned with ISO 45001:2018 and ISO 31000:2018. This study concludes that the integrated model effectively improves risk management and can be replicated in similar industrial activities.
